Ultimate Benefits Plan Member Installation Checklist

***Please follow these in the order they appear. Each one is as important as the next, do not skip***

• PRINT THIS FORM OUT AND KEEP IN AN EASILY SEEN PLACE. CHECK THE BOX WHEN EACH ACTIVITY IS COMPLETE.

• Once online sign up is complete, go to the UBP member manual - 55 pages, and if you are going to take advantage of the business opportunity also the business opportunity manual - 23 pages, and PRINT them out. These are on the www.cmiubp.com (bookmark) website under the getting started - online sign up page at the top, click section C at the top for the manuals link and it will take you to both of them.

Read the manuals, all pages.

• Go to the www.cmiubp.com/csa website (bookmark for future reference, as this is the official UBP training website). Click UBP conference calls at the top. That will take you to a page that has "Training Sessions" at the top. There are 11 recorded training calls that you need to listen to:

• Gather your declarations pages for your homeowners, auto, life, and health insurances so you know what the exact coverage you have now for each of them is. Then call Answer Financial for your auto, homeowners, and life insurance at 800-233-3028, and get quotes for what their prices are for the SAME coverage as you have now. If they are less, switch. Also remember there is a $500 auto deductible coverage you get as soon as you sign up in the UBP so it MAY make sense for you to increase your deductible to $1000 if you haven't already and still have a $500 true deductible but pay a lower monthly premium. There are a couple ways to utilize this benefit.

• For a health insurance quote go to www.cmiubp.com click the benefits play button at the bottom of the page. Click the box choice that applies to you. Click the health benefits box. (For pre-existing conditions and guaranteed acceptance get with your UBP associate and they will walk you though that process) Click Major Medical, and then click the box that applies to you. Then at the bottom of that page there is a link to e-health insurance which will tie in the code that you are a UBP associate and give you the rate as part of the group plan. Follow the instructions from that point forward to get your coverage quote. If you have questions about this please ask as health care benefits can be confusing sometimes.

• Once your USA Lifestyle + card comes in the mail, activate it, and view the disk that comes with it that has the lifestyle plus manual on it, print it out, and read it.

Read the lifestyle plus manual completely, and visit the www.usahc.com (bookmark) website.

• You will receive your Virtual Money ATM card a few weeks after sign up. Once received, activate it and fund it right away. If you need assistance with how to fund go to the www.cmiubp.com website and on the left of the home page click member benefits. It will give you options on how to load it, or you can ask any UBP associate available. ***It is crucial you do this the second you receive your card, not only will this serve as the way you send/receive funds to and from CMI, but it also activates another health benefit card, as well as this is now your internal bookkeeping system for ALL of you business related expenses***

• Talk with your CPA or tax preparer and get educated on the tax benefits of having your new home based business, and possible adjustments to your withholdings if you are a w-2 wage earner, to receive more of your paycheck now, without having to pay any taxes at the end of the year. This also applies to self employed individuals but will be most noticed at the end of the year. ***Please note this is not tax advice only a recommendation to consult a tax professional to advise you the best ways to utilize tax advantages for your specific situation most effectively***         

• USE THE BENEFITS!! Keep in mind it will most likely take 90-120 days total for you to completely be integrated and receive all the membership cards associated with the UBP benefits. At that point you will be highly educated on all of the benefits in the UBP, and be using them and watching more and more money show up in your bank account each month, without generating a single dime of income more than before you were with the association.
